The Tom who is married to Catherine in the flashback here is not the same Tom Gunther who appeared in the pilot episode of the series
Just before Vincent falls into his delirium, he throws a chair through his stained glass window next to his bed. When he awakens, the window has miraculously repaired itself.
Ironically, the only character in this episode to show Vincent what the world would be like without him that did not radically change was Paracelsus, who is apparently evil in all possible worlds.
